Whether you're an actor or a volunteer, we need these filled out before you attend.
Miracles Forms....
Our Actor Registration form helps us know more about our actors and their specific needs! This provides essential information ranging from how to spell their name for the program, to media release! All sensitive information will be kept confidential.
Our Code of Conduct and Ethics form is essential for all involved to complete. It lays out our ground rules and clearly sets expectations for rehearsals.
Our Volunteer Agreement and Release of Liability form is very important to complete. Volunteering with vulnerable populations requires a background check, be prepared for more info on that.
